Instruction Letter to Client – West Palm Car Wreck
Dear Client:
While you are treating for your injuries it is imperative that you keep me informed of your progress. It is also important to maintain a relationship with the treating medi¬cal personnel. With that in mind, please do the following:
1. Keep all medical appointments such as doctors, physical therapists, and labora¬tory unless you absolutely have to cancel. Failure to keep appointments can be used against you in your case.
2. Be sure to tell your doctor and medical personnel accurately about your symp¬toms and your injuries. Let them know what causes your good days, your bad days, your difficulties, and your pain.
3. Do not tell the doctor you are much better if, in fact, you are not. Be honest with your doctor.
4. If you relapse after periods of recovery be sure to visit the doctor and let him know about these relapses if the aggravation is fairly serious.
5. Be accurate with your therapist about the locations of your pain and disability.
6. Keep your doctor and therapist informed about how your injury affects:
A. Your work.
B. Your leisure activities.
C. Your emotional well being.
D. Your relationship with your spouse and family.
7. If you had a previous injury to the same area of your body be sure to tell the doctor and therapist completely about the previous injury including:
A. The nature of the injury.
B. How the injury occurred.
C. When you obtained recovery.
D. The extent of recovery.
E. How this present injury (from your automobile accident) differs from the first injury.
8. Do what your doctor and therapist tell you. If they instruct you to exercise and apply heat, then do as they ask. Failure to follow their advice will be used against you in your case.
9. Review the doctors' reports that we furnish to you. If they are incorrect, notify me so that I may instruct you as to how to inform the doctor.
10. Do your absolute best to recover as quickly as possible as the law requires you to mitigate your own damages.
