1. My job as the attorney is to process your claim and your first job is to recover from your injury.
2. Do not talk to anyone about this case with the possible exception of your immedi¬ate family members and members of this office. Do not, under any circumstances, dis¬cuss the case with potential witnesses, insurance claim representatives or even your friends as certain things that you say may be misconstrued or even used against you.
3. Be sure to keep your medical appointments and be accurate in discussing your in¬juries with medical personnel.
4. Keep my office informed as to your recovery progress and the names of all doctors, physical therapists, or other medical personnel that you see.
5. Maintain a separate file regarding this case in which you keep the following items in a safe place:
a. All correspondence and documents from the attorney's office.
b. A list and/or copies of all medical bills, prescription bills, and any other ex¬penses related to your injury.
c. A separate record of lost wages or loss of income as explained below in #6.
d. A list of all witnesses as discussed below in #10.
e. All containers in which medications are provided.
f. Copies of negatives of all pictures regarding this incident so that blowups can be made, if necessary, prior to settlement or trial.
g. Copies of your diary which we have discussed.
6. Keep a separate record of your time lost from work or loss of income so that it can be compared with lost wage statements which we will obtain from your employer. This request is absolutely imperative because your lost wages and lost income are crucial factors in your case.
7. Make sure our office receives a copy of each and every medical bill incurred even if it is paid by your health insurance carrier, workers' compensation carrier, or some other party. If you buy prescription medicine, be certain to retain the receipt.
8. Maintain a separate record of medical bills in your own file so it can be compared with our record before settlement discussions.
9. If your injury is one that is visible such as a scar, disfigurement, broken leg, etc., have pictures taken on a biweekly basis so a pictorial record can be kept of such dam¬ages.
10. Maintain a list of the witnesses in your file and request they notify you of any change in address, telephone number, etc.
11. Notify us of any additional witnesses, change of address of witness, or any other important information regarding witnesses who have either seen the occurrence which is the subject of this claim or who are witnesses to your pain and suffering.
12. It is absolutely imperative that you advise me or this office of any change in ad¬dress, telephone number, marital status, or employment. This request cannot be over¬emphasized. A delay or failure to contact us regarding such changes could seriously ef¬fect the value of your case.
